The University Injury/Incident/Hazard Report Form must be completed for all injuries and the appropriate page forwarded to Health & Safety, Human Resource Services within 24 hours. If you are using the new form please remember to retain copies as appropriate.

Note significant injuries/dangerous occurrences must be notified to WorkCover within 48 hours. Health & Safety will assist you with this if you contact them on ext. 15263.
The register is issued to FAOs and/or school secretaries. Full registers must be returned to Health & Safety and a replacement will be issued. To ensure confidentiality, the personal details section on the last page (white page) has been specially treated to not reproduce.
If using the new Incident Report Form please remember to retain copies as appropriate.
The register can be used as your treatment record to prevent duplication of records.
