The official proposal for a Program takes one of two forms, either the Pro Forma OR Program Tracking System (PTS). The information below lists the appropriate form for data entry and official proposal for each type of program.

The Pro Forma is a Word Document used for the new and revised programs as well as existing programs: refer to the table above.
These programs will remain in Pro Forma format and will not be stored in PTS. The Pro Forma contains hyperlinks to detailed background information to assist you. Please click in the table for a Pro Forma template.
PTS is an online system used for entering new Programs and revised programs as well as existing Programs. You will either:
If your active program is not in PTS, you will be able to migrate existing Program information into PTS; contact your Faculty QA, who will show you how to do this.
Note: existing program data will be migrated over time as reviews and/or revisions take place. Non Award Program and Summer School do not use Pro Formas or PTS.
